Role Overview: Provide outpatient care focused on the prevention, early detection, and management of osteoporosis and other metabolic bone conditions. Play a key role in the Bone Health and Fracture Prevention Program, delivering comprehensive assessments, risk evaluations, and personalized care plans to reduce fracture risk and improve long-term bone strength. Collaborate with physicians, physical therapists, and other specialists to develop holistic treatment strategies that promote mobility, independence, and quality of life. Educate patients and families on lifestyle modifications, nutrition, fall prevention, and medication management to support bone health. Participate in care coordination, follow-up, and interdisciplinary case reviews to ensure continuity of care and optimal patient outcomes. Schedule: Monday: 8:00 AM – 12:00 PM Tuesday & Wednesday: 8:00 AM – 4:30 PM No weekends, call or Holidays.
