About the position
The RN Admission Nurse at Legacy Home Care is responsible for performing Start of Care assessments for clients in the Twin Cities area. This role involves creating personalized care plans and coordinating with a collaborative team to ensure clients receive appropriate care. The position offers flexibility in work hours and the opportunity to make a significant impact on patient care.
Responsibilities
- Conduct thorough OASIS admission assessments
- Develop individualized care plans for new clients
- Suggest additional services as needed to support holistic care
- Communicate with clients, their families, physicians, and care team members
Requirements
- Active RN license in Minnesota
- Reliable vehicle with insurance and a valid driver's license
- Experience with Medicare and OASIS documentation
- Ability to work independently and manage care with excellent organizational skills
