About the position

The Admissions Coordinator at Pacific Coast Post-Acute Care plays a crucial role in facilitating the admissions process for students and residents. This position is focused on maintaining high medical integrity while fostering a supportive team environment. The coordinator will be responsible for efficiently admitting residents in accordance with established policies and procedures, ensuring a smooth transition for new residents into the facility.

Responsibilities

  • Efficiently and timely admit residents according to established policies and procedures.
  • Interview residents, guardians, and sponsors to obtain required admission information and signatures.
  • Prepare identification records for residents as per established policies and procedures.
  • Maintain an accurate record of available beds.
  • Admit, transfer, and discharge residents in adherence to established policies and procedures.
  • Provide residents with admission information packets, including resident rights, notice of privacy practices, and admissions contract.

Requirements

  • Experience in Skilled Nursing Facility (SNF) preferred.
  • Motivated and hardworking attitude.
  • Eager to grow in the healthcare industry.
