About the position

Cross Keys Village - The Brethren Home Community is searching for an experienced Licensed Practical Nurse to fulfill the role of the Admission & Discharge Nurse to provide support to the clinical team within the Mission Point Health Care Center. This opening is for a full-time position working Monday through Friday. Occasional evening, night and weekend hours may be required. Must have the ability to work a flexible schedule as needed.

Responsibilities

  • Facilitating all aspects of the admission and discharge process for residents and rehab guests by assuring all components are thoroughly, timely and accurately completed according to facility guidelines and resident/guest needs.
  • Evaluating the health status of all residents and rehab guests upon admission and discharge.
  • Maintaining exemplary customer service to residents, guest and families.
  • Assuring a smooth transition for all new and discharging residents and rehab guests.
  • Coordinating any equipment and supply needs prior to admission.
  • Ensuring all items are taken upon discharge and the residents belongings form is completed with the responsible party signature.
  • Working with the Admission Department for potential referrals/admissions and serving as a resource for both departments.
  • Working with social services for upcoming discharges and ensuring discharge paperwork is completed.
  • Obtaining admission and discharge orders and communicating resident and rehab guest needs to the neighborhood team as well as with the physician to make he/she aware of the admission and discharge.
  • Completing evaluations as needed to meet mandated requirements when there are no scheduled admissions or discharges.
  • Demonstrating quality-mindedness, critical thinking, and proactive initiative in leadership responsibilities, interpersonal sensitivity, and communication skills.
  • Participating in nursing/medical care audits/studies.

Requirements

  • Must be licensed as a Licensed Practical Nurse in the State of Pennsylvania.
  • Must demonstrate CPR competencies.
  • Must be open to continuing education as required per community standards and state/federal regulations and to enhance professional development.
