About the position

The Acute Nurse Case Manager (RN) is a hybrid role that involves both in-person visits to local hospitals and remote work. This position focuses on achieving positive patient outcomes and managing quality of care across various settings, including acute and post-acute facilities. The role emphasizes patient advocacy, care coordination, and collaboration with healthcare teams to develop effective care plans.

Responsibilities

  • Manage and plan transitions of care, discharge, and post-discharge follow-up for patients admitted to key hospitals.
  • Establish trusting relationships with patients and their caregivers.
  • Collaborate with clinical staff to develop and execute care plans and achieve goals.
  • Report variations to primary care physicians and implement appropriate actions.
  • Build relationships with preferred acute care providers and direct referrals to them.
  • Coordinate integration of social services and case management functions across various healthcare settings.
  • Facilitate patient/family conferences to review treatment goals and optimize resource utilization.
  • Serve as a patient advocate and enhance collaborative relationships to maximize informed decision-making.
  • Conduct concurrent medical record reviews and evaluate patient satisfaction and quality of care provided.

Requirements

  • Associate degree in Nursing required with a valid, active RN license in PA.
  • Minimum of 2 years' clinical work experience required.
  • Minimum of 1 year of utilization review and/or case management experience required.
  • Certified Case Manager certification preferred.

Nice-to-haves

  • Bachelor Degree in Nursing (BSN) or RN with Bachelor Degree in a related clinical field preferred.
  • 1 year of case management experience in acute or community case management highly desired.
